This form should be used to submit data for vouchers issued by the FMC (i.e. those requested up to and including 31 July 2023, with a reference number starting MOJxxxxx).
It is a condition of taking part in this scheme that this form is completed, though it is optional for clients to complete the monitoring forms.
The voucher scheme provides valuable information which can be used to help shape future policy.
